INSURANCE/FEES

Ponte Vedra Neuropsychology works with you to make your care accessible and affordable.

In-Network:
Traditional Medicare only. We bill your Medicare directly for neuropsychological evaluations and therapy. 

We are out of network with all Medicare Advantage plans, though will submit claims on your behalf. It is variable what out of network benefits plans will cover and we encourage individuals to contact their insurance to inquire about out-of-network benefits for the CPT codes listed below. Always request the information in writing from your insurance. We place a credit card on file at scheduling (no upfront charge). You'll pay only after your insurance processes the claim, typically 3-6 weeks after your evaluation is complete.

Private Pay/Out-of-Network:
Evaluations:  $1,000–$2,500
Psychotherapy Intake: $250
Therapy Session: $225/session
Brain Health Coaching: $250/hour

Payment is due at time of service for private pay services. We use the HIPAA-compliant system Ivy Pay to collect payment. Ivy Pay works with credit, debit, HSA, and FSA cards. It's HIPAA-secure, keeps the sessions confidential, and makes payment easy. We enter the bill amount into the system and Ivy Pay sends you a text message, asking you to enter the payment information (all credit, debit, HSA, and FSA cards are accepted). 

Good Faith Estimates and SuperBills to submit to your insurance for out-of-network benefits, are provided upon request.

Questions to Ask the Insurance Company

• Does the plan offer out-of-network benefits for neuropsychological testing (CPT codes 96116, 96121, 96132, 96133, 96138, 96139) or psychotherapy services (90791, 90837)?

• Is prior authorization required?

• Is a referral from a physician required?

• What is the copay, coinsurance, and/or deductible for these services?

• How much of the deductible has been met for the year?

• Is there a cap on the number of testing units or hours covered per year?

• Are there any plan exclusions for neuropsychological testing?
